Art Appraisals?

 I am donating a piece of artwork to charity and they require an art appraisal in order to issue me a tax receipt. I have an artists friend and gallery owner who may be able to do the appraisal for me but I need a good blank appraisal form to provide him with.  Does anyone know where to find good sample art appraisal forms? I did a quick search and havn't really found anything yet. I am looking for something with the appropriate categories that we can simply fill in.

I have a few things to talk about...

Hello guys hope all is well in the LMM world. As for myself I am doing good. Hurricane Irene canceled my show I was having but it has been postponed until October. And while I was riding passenger in a utility van we had a 5.6 earthquake here on the east coast of the USA prior to the hurricane and that never happens and I missed it. They say if you were driving you would not have felt it or realized it was an earthquake.

Found Petroglyph (Update)

Found Rock Petroglyph

Unless I have completely gone insane, I am quite certain that the marks on the rock were carved by hand. The question is: were these marks made recently or do they travel back many centuries? By the way...I took the rock with me. I did some imagery searching online and came across a reindeer petroglyph that looks like the one on this rock. Shaping the future

Since my husband's death, I have been re-evaluating my life. In a way, his passing brought to a close the life I knew, and I am having to build a new life without him. I have realised that I can view this as a blessing or a curse and have chosen to see this as an opportunity (albiet painful one) to recreate myself and choose the life I want instead of letting circumstances dictate it for me.

I have never really learned how to dream or set goals for myself, so I am on a steep learning…

This might be good for...

I've been through some heavy crap lately.  I won't go into the gory details, but I tell you, it can be life altering.  I am trying to get a foothold - and I turned my attention (finally) back to art to help me cope.

I always had this view of artists in their studios, with their easels and paint, supplies in nice arty boxes, maybe an oriental rug on the floor, stained glass in the windows, listening to wonderful music as they create.  Somehow, I felt this was an achievable model…

So once i have my image printed onto transparency film, I take my Ezscreen silk screen which has been coated in light sensitive emulsion (These sheets come pre-coated so they are ready to use). Place it onto a black board, then place the transparency image on top (this part needs to be done in a dark room with no direct sunlight). I take it outside and expose it for 1 minute and 40 seconds (but time varies depending on time of day and strength of sunlight). After i have exposed the image i place it into a tub of water for around 5 - 8 minutes. Then i rinse the screen out under fast running water until the unexposed area (the black) washes away. I then expose the screen to the sun for a further 20mins to completely harden the light sensitive emulsion. 

And this is the completed screen ready for printing.  I simply use an old credit card to spread the ink over the screen making sure i do one complete full stroke across the screen (holding the credit card at a 45 degree angle). I immediately wash the screen out under fast flowing water and pat dry. I use fabric screen printing ink which washes out in water (which makes the process easy).
